This book gives a brief overview of the "Patient Blood Management" (PBM) concept and is addressed to health professionals as well as all persons generally involved into hospital management. "Patient Blood Management" (PBM) is a multidisciplinary, evidence-based approach to improve perioperative care. The concept is based on three pillars that target modifiable risk factors for adverse outcome: i) preoperative screening, diagnosis and treatment of anaemia, ii) blood-sparing techniques, and iii) adequate blood product utilization. The superior objective is to optimize patient outcome. Anaemia is common among hospital patients and an underestimated risk factor for blood transfusions, higher morbidity and mortality. Optimizing and preserving red cell mass before, during and after surgery can be achieved by a number of measures that can be implemented in EVERY hospital. These measures have high potential to improve patient outcome and to reduce costs both for allogeneic blood transfusions and for the treatment of the associated complications. Furthermore, due to the current demographical development, the offer of available blood products is most likely to decrease in the future whilst advances in modern medicine increase blood product demands, thus causing scarcity. Therefore, it is our duty towards society, patients and donors to use blood products wisely and only when absolutely necessary. Patient Blood Management is our strategy to face this challenge. Further, since the field of medicine, both from a scientific and regulatory perspective, rapidly changes, the readers are advised to continuously update their knowledge by attending national meetings and reading clinical journals. To equip the readers with the basic knowledge in critical reading and data analysis, which is an essential skill in daily medical practice, a novel chapter titled “Data Interpretation in Laboratory Medicine was included in this book. In this chapter, the readers are asked to make logical conclusions based on the given data and/or statistical results. Moreover, there is also a chapter on “Practical Calculations in Transfusion Medicine, Apheresis, and Hemostasis to help consolidate all the necessary formulas commonly used in daily practice for easy reference. These chapters are unique to our book and will not be found in any other currently on the market. All of the questions in this book were originally created by the authors of each chapter. Each question can either be standalone or part of a case scenario representing challenge cases in Transfusion Medicine, Apheresis, and Hemostasis. These questions often represent both rare and common clinical scenarios that the authors have seen during their clinical practice. Each question is then followed by 5 possible answers, with only one being correct (or the best answer). After the question, there is a conceptual explanation followed by a more factual explanation of the right and wrong answers. We gave the individual authors the freedom to choose how they explained the wrong answer choices. Some authors chose to be more direct (e.g. Answer A is incorrect because...), while other authors chose a more conversational style (e.g. Human resources (answer A) includes staffing, selection, orientation, training, and competency assessment of employees). This format is designed to help the student linking the conceptual and factual knowledge together to form a solid foundation for use in clinical practice. At the end of each chapter, there is a list of articles and textbooks that will prove useful to the motivated student who wishes to become an expert in the field. Another special feature to our textbook is the presence of a pre-test and post-test, which are provided to help the readers with self-assessment. As stated above, the main focus of this book is to help the readers preparing for the ABP certification exam in Transfusion Medicine. Patient Blood Management (PBM) has been increasingly recognized as a fundamental element of good clinical practice in transfusion. The importance of implementing effective PBM had been highlighted in the WHA 63.12 (2010) and reconfirmed in the WHO Action Framework to advance universal access to safe, effective and quality assured blood products 2020-2023 which was launched in February 2020. This policy brief aims: (1) to raise awareness of health authorities and other governmental agencies on the benefits of PBM in improving the population health status, patient outcomes, patients’ safety and quality of care, improving clinical transfusion practice while saving costs; (2) to emphasize governments should take the leadership roles, including allocating sufficient funds to create the structural needs to enable nationwide PBM implementation; and (3) to encourage effective incorporation of PBM into daily clinical practice by creating awareness in patients at large and their health care providers, that anemia and/or moderate to severe blood loss are serious predictors of adverse outcomes. The target audience of this policy brief is government representatives/health authorities, executives of health services and hospitals, health care practitioners, other stakeholder groups including private and public funders/payers, medical schools, professional societies, patient organizations, quality and safety regulatory and accreditation organizations.

To reduce transfusion-related morbidity and mortality, it is recommended that an integrated approach to blood management is employed using all available tools to reduce a patient's exposure to donor blood.
