An exploration of the much-derided English suburbs through rap music. There are many different Englands. From the much-romanticized rolling countryside, to the cosmopolitanism of the inner cities (embraced by some as progressive, multicultural enlightenment and derided by others as the playground of a self-righteous metropolitan elite), or the disparagingly named "left behind" communities which, post-Brexit, have so interested political parties and pundits, demographers and statisticians. But there is also an England no one cares about. The England of semi-detached houses and clean driveways for multiple cars devotedly washed on Sundays, of "twitching curtains" and Laura Ashley sofas; of cul-de-sacs to nowhere and exaggerated accents; of late night drives to petrol stations on A roads, fake IDs tested in Harvesters, and faded tracksuits and over-gelled hair in Toby Carverys; of questionable hash from a "mate of a mate" and two-litre bottles of White Lightning from Budgens consumed in a kids playground. Much derided. Unglamorous, ordinary; cultural vacuity and small "c" conservatism. A hodgepodge. An—apparently—middling, middle-of-the-road middle-England of middle-class middle-mindedness. Part poetry anthology, part academic study into placemaking, and part autoethnography, The England No One Cares About innovatively brings together academic discussions of the ethnographic potential of lyrics, scholastic representations of suburbia, and thematic analysis to explore how rap music can illuminate the experiences of young men growing up in suburbia. This takes place by exploring the author’s own annotated lyrics from his career as a musician known as Context where he was referred to by the BBC as "Middle England’s Poet Laureate."

First published in 1998, the aims of this book are: the comparison of community care service and financing systems, the comparison of reform arguments and history over the last decade, the comparison of who uses how much of what services, and with what impact on their needs and the probability of having to enter institutions for long-term care. The book breaks new ground by comparing systems from a new perspective and describing contemporary reform argument and proposals for the first time in the English language. It presents new evidence from the most ambitious collection and analysis of quantative data so far made for the comparison of the two countries (based on matched area samples collecting comparable information about cohorts of new users on two or more occasions). The book also shows how the need-related circumstances of users differ between countries and within each country between areas. The book shows how and why higher levels of the French cash benefit for community care had more effect on the central policy goal than its British counterpart, how higher levels of services generally had little impact on it in either country, but on average, how the effect of the British services were much greater.

Care for patients who have suffered major trauma, for example following a road accident or a fall, has not significantly improved in the last 20 years despite numerous reports identifying poor practice, and services are not being delivered efficiently or effectively. Survival rates vary significantly, with a range from five unexpected survivors to eight unexpected deaths per 100 trauma patients, reflecting the variable quality of care. 450 to 600 lives could be saved each year in England if major trauma care was managed more effectively. For best outcomes care should be led by consultants experienced in major trauma; but major trauma is most likely to occur at night and at weekends, when consultants are not normally in the emergency department. Major trauma care is not coordinated and there are no formal arrangements for taking patients directly for specialist treatment or transferring them between hospitals. A significant number of patients that need a scan CT do not receive one. Not enough patients who need a critical care bed are given one. Access to rehabilitation services varies and patients are not always receiving the care that they need. The estimated annual lost economic output from deaths and serious injuries from major trauma is between £3.3 billion and £3.7 billion. Only 60 per cent of hospitals delivering major trauma care contribute to the Trauma Audit and Research Network (TARN). The performance of the 40 per cent of hospitals that do not submit data to TARN cannot be measured.

The role of pastoral care reconsidered in the context of major changes within the Anglo-Saxon church. The tenth and eleventh centuries saw a number of very significant developments in the history of the English Church, perhaps the most important being the proliferation of local churches, which were to be the basis of the modern parochial system. Using evidence from homilies, canon law, saints' lives, and liturgical and penitential sources, the articles collected in this volume focus on the ways in which such developments were reflected in pastoral care, considering what it consisted of at this time, how it was provided and by whom. Starting with an investigation of the secular clergy, their recruitment and patronage, the papers move on to examine a variety of aspects of late Anglo-Saxon pastoral care, including church due payments, preaching, baptism, penance, confession, visitation of the sick and archaeological evidence of burial practice. Special attention is paid to the few surviving manuscripts which are likely to have been used in the field and the evidence they provide for the context, the actions and the verbal exchanges which characterised pastoral provisions.

Approximately 9 million patients receive urgent primary out-of -hours care in England. In April 2004 the Department of Health gave GPs the chance to opt out of providing this service and transfer responsibility to the Primary Care Trust. This report looks at three main issues related to the change: how well did the Department of Health prepare; how did the new service perform and what did it cost. It concludes that the preparation was shambolic both at local and national level and although the new service is starting to improve performance against key access targets is still not good enough. In addition the cost of the new out-of-hours service has been £70 million higher than was foreseen.
