On February 23-25, 2021, a planning committee convened by the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 3-day virtual workshop titled Systematizing the One Health Approach in Preparedness and Response Efforts for Infectious Disease Outbreaks. The workshop gave particular consideration to research opportunities, multisectoral collaboration mechanisms, community-engagement strategies, educational opportunities, and policies that speakers have found effective in implementing the core capacities and interventions of One Health principles to strengthen national health systems and enhance global health security. This publication summarizes the presentation and discussion of the workshop.

In November 2018, an ad hoc planning committee at the National Academies of Sciences, Engineering, and Medicine planned two sister workshops held in Washington, DC, to examine the lessons from influenza pandemics and other major outbreaks, understand the extent to which the lessons have been learned, and discuss how they could be applied further to ensure that countries are sufficiently ready for future pandemics. This publication summarizes the presentations and discussions from both workshops.

Emerging infectious disease threats that may not have available treatments or vaccines can directly affect the security of the world's health since these diseases also know no boundaries and will easily cross borders. Sustaining public and private investment in the development of medical countermeasures (MCMs) before an emerging infectious disease becomes a public health emergency in the United States has been extremely challenging. Interest and momentum peak during a crisis and wane between events, and there is little interest in disease threats outside the United States until they impact people stateside. On March 26 and 27, 2015, the Institute of Medicine convened a workshop in Washington, DC to discuss how to achieve rapid and nimble MCM capability for new and emerging threats. Public- and private-sector stakeholders examined recent efforts to prepare for and respond to outbreaks of Ebola Virus Disease, pandemic influenza, and coronaviruses from policy, budget, and operational standpoints. Participants discussed the need for rapid access to MCM to ensure national security and considered strategies and business models that could enhance stakeholder interest and investment in sustainable response capabilities. This report summarizes the presentations and discussions from this workshop.

Close contact between people, animals, plants, and their shared environment provides more disease transmission opportunities. Host characteristics, environmental conditions, and habitat disruption can provide new opportunities for disease to occur. These changes may lead to the spread of existing and new diseases. Bacteria, viruses, fungi, protozoans, sporozoans, worms, and others cause infectious diseases. Some of these diseases may be prone to explosive outbreaks and may constitute deadly epidemic threats that could rapidly reach pandemic proportions. Drugs and vaccines can successfully control many infectious diseases; however, this is challenged by the lack of facilities and resources. In all parts of the world, infectious disease is an essential constraint to increased human, animal, and environmental interactions. Identifying hot-spot and interventions for prevention while considering the heterogeneity of target diseases to places, population time, or situation is essential. Therefore, successful infectious disease control measures must be based on understanding disease transmission pathways, strengthening surveillance systems, and intervention. Application of the One Health method is a responsive approach to infectious disease control. Much of the One-Health based approach to managing an infectious disease has been utilized with a promising effect on controlling current outbreaks. More deliberate efforts should encourage understanding of disease determinants to analyze infectious disease issues through a One-Health lens. Only through the extensive participation of all related field stakeholders can One-Health truly reach its potential to mitigate infectious disease outbreaks. This chapter reviews utilization of the One Health approach to infectious disease outbreak control.

Proposes the pragmatic changes we must make to survive COVID and the worst of the new diseases on the horizon The Trump administration’s neglect and incompetence helped put half-a-million Americans in the ground, dead from COVID-19. Joe Biden was elected president in part on the promise of setting us on a science-driven course correction, but, a little more than a year later, another half-a-million Americans were killed by the virus. What happened? In The Fault in Our SARS, evolutionary epidemiologist Rob Wallace catalogs the Biden administration's failures in controlling the outbreak. He also shows that, beyond matters of specific political persona or party, it was a decades-long structural decline associated with putting profits ahead of people that gutted U.S. public health. COVID-19 isn’t just an American tragedy. Each in its own way, countries around the world following the "profit-first" model failed their people. Global vaccination campaigns were bottled up by efforts to protect pharmaceutical companies' intellectual property rights. Economies were treated as somehow more real than the people and ecologies upon which they depend. Frustrated populations pushed back against lockdowns, abuses of governmental trust, and, fair or not, the very concept of public health. A social rot meanwhile wended its way into the heart of the sciences that, tasked with controlling disease, serve the systems that helped bring about COVID-19 in the first place. In The Fault in Our SARS, Wallace and an array of invited contributors aim to strip down the capitalist social psychology that in effect protected the SARS virus. The team proposes instead new approaches in health and ecology that appeal both to humanity's highest ideals and the pragmatic changes we must make to survive COVID and the worst of the new diseases on the horizon.

Immunization against disease is among the most successful global health efforts of the modern era, and substantial gains in vaccination coverage rates have been achieved worldwide. However, that progress has stagnated in recent years, leaving an estimated 20 million children worldwide either undervaccinated or completely unvaccinated. The determinants of vaccination uptake are complex, mutable, and context specific. A primary driver is vaccine hesitancy - defined as a "delay in acceptance or refusal of vaccines despite availability of vaccination services". The majority of vaccine-hesitant people fall somewhere on a spectrum from vaccine acceptance to vaccine denial. Vaccine uptake is also hampered by socioeconomic or structural barriers to access. On August 17-20, 2020, the Forum on Microbial Threats at the National Academies of Sciences, Engineering, and Medicine held a 4-day virtual workshop titled The Critical Public Health Value of Vaccines: Tackling Issues of Access and Hesitancy. The workshop focused on two main areas (vaccine access and vaccine confidence) and gave particular consideration to health systems, research opportunities, communication strategies, and policies that could be considered to address access, perception, attitudes, and behaviors toward vaccination.
