It has now been 25 years since the apocryphal report in the CDC Morbidity and Mortality Weekly Report dated June 5, 1981 entitled, “Pneumocystis Pneumonia - Los Angeles”, which announced what was to become HIV/AIDS. HIV has now affected virtually all countries that have looked for it and has had a devastating impact on the public health and medical care infrastructure around the world. HIV/AIDS has also disproportionately affected nations with the least capacity to confront it, especially the developing world nations in Sub-Saharan Africa, South and Southeast Asia, and the emerging republics of Eastern and Central Asia. The pandemic, unlike any other disease of our time, has had profound impacts on the practice of public health itself: bringing affected communities into decision making; demanding North-South partnerships and collaborations; and changing the basic conduct of clinical and prevention trials research. While much has been written in scholarly publications for medical, epidemiologic and disease control specialists, there is no comprehensive review of the public health impact and response to HIV/AIDS in the developing world. This edited volume seeks to systematically describe the emergence and form of the epidemics (epidemiology), the social, community and political response, and the various measures to confront and control the epidemic, with varying levels of success. Of particular importance are strategies that appear to have been useful in ameliorating the epidemic, while contrasting the situation in a neighboring country or region where contrasting prevention or care initiatives have had a deleterious outcome. Common to all responses has been the international multi-sectoral response represented by the Global Fund for HIV/AIDS, Malaria and Tuberculosis, the President's Emergency Plan for AIDS Relief, and the Gates Foundation, among others, to promote HIV pharmacologic therapy in resource-poor settings. The chapter authors will explore the political challenges in meeting HIV/AIDS prevention and care in concert with the public health realities in specific country and regional context.

In highlighting how a WHO Public Health Approach (PHA) has been successfully used in developing countries to provide HIV/AIDS patients with antiretroviral therapy (ART), this important book provides a template for how the PHA can be implemented to treat other chronic but non-communicable diseases (NCDs) as well. With over 28 million people globally now receiving treatment for HIV/AIDS, it’s clear there are lessons to be learnt from the provision of ART which have great relevance for NCD care and towards achieving universal health coverage in the global south. The first section of the book provides a detailed overview of the strategy that enabled such a successful programme to be taken place, the challenges faced and its evolution over time. The book then moves on to assert that by approaching other chronic NCDs in a similar way, focussing on populations with integrated long-term and short-term person-centred care, there is a pathway towards universal health care and Universal Health Coverage across the developing world. Discussing many of the most pressing diseases and public health issues affecting these regions, this book provides global health scholars and practitioners with a detailed analysis of the challenges faced in tackling these diseases, but also an integrated person-centred health-care approach by which these challenges may be met.

We began to research for this book in 2000, with the idea that we might contribute to the search for solutions to the global HIV/AIDS pandemic by c- bining perspectives from different disciplines. Much has happened in the interv- ing years. First, the severity of the HIV/AIDS pandemic in sub-Saharan Africa – and the threat it posed for many others regions of the world – led to a movement among several countries to correct the imbalance between producers and users of ph- maceutical products. This effort produced a clarification of the right of gove- ments to produce generic medicine under compulsory licenses and an amendment of the World Trade Organization’s TRIPS Agreement to allow exports of generic medicines from one WTO Member to another. In 2007, the amended rules were put into practice, with Canada authorizing the export of generic antiretroviral drugs to Rwanda. However, at the same time, global patent laws have been undermined due to regulatory capture, most notably in free trade agreements and through political pressure on countries like Thailand to not to exercise their right to issue compulsory licenses for pharmaceutical products. Second, the amount of money available for the treatment and prevention of HIV/AIDS has increased dramatically, with the establishment of the World Bank Multi-Country HIV/AIDS Program for Africa (MAP), the Global Fund to Fight AIDS, Tuberculosis and Malaria and the US President’s Emergency Plan for AIDS Relief (PEPFAR), among other funding initiatives.

Gender issues are central to the causes and impact of the ongoing AIDS epidemic. The editors bring together cutting edge contemporary scholarship on gender and AIDS in one volume. They address questions related to gender and sexuality, how women and men live the epidemic differently and how such differences lead to different outcomes. The volume joins research on Africa, Asia and Latin America and illustrates how the epidemic has different gendered characteristics, causes and consequences in different regions. Collectively, the chapters demonstrate the fundamental ways that gender influences the spread of the disease, its impact and the success of prevention efforts.
