Globalization and economic progress has been accompanied with an increase in the incidence of obesity, cardiovascular disease and other non-communicable illnesses worldwide among populations in some countries. All aspects of illness and healthcare are mediated by language: experiences of illness, death and healthcare provision are talked and written about (face-to-face or online), while medical consultations, research interviews, public health communications and even some diagnostic instruments are all inherently linguistic in nature. How we talk to, about and for each other in such a sensitive context has consequences for our relationships, our sense of self, how we understand and reason about our health, as well as for the quality care we receive. Yet, linguistic analysis has been conspicuously absent from the mainstream of medical education, health communication training and even the medical or health humanities. The chapters in this volume bring together applied linguistic work using discourse analysis, corpus methods, conversation analysis, metaphor analysis, cognitive linguistics, multiculturalism research, interactional sociolinguistics, narrative analysis, and (im)politeness to make sense of a variety of international healthcare contexts and situations. Obesity is a global ticking time bomb with huge potential negative economic and health impacts, especially for the poor. As of 2016, an estimated 44 percent of adults (more than two billion) worldwide are overweight or obese, and over 70 percent of them live in low- or middle-income countries, dispelling the myth that obesity is a problem only in high-income countries. The global obesity epidemic presents a formidable challenge to human capital acquisition, national wealth accumulation, and the goals of ending extreme poverty and boosting shared prosperity. Given the renewed global focus on human capital, its links to the obesity epidemic, and the growing evidence base for double- and triple-duty actions, there is both an urgent need for action and a great opportunity for engagement that will require both a whole-of-government and a whole-ofdevelopment-partner approach. Countries and global partners need to act urgently to address this ensuing epidemic, with emphasis on interventions that require corrective public action rather than one of individual responsibility.

The Roundtable on Obesity Solutions of the National Academies of Sciences, Engineering, and Medicine held a workshop in Washington, DC, on April 6, 2017, titled The Challenge of Treating Obesity and Overweight: A Workshop. The discussions covered treatments for obesity, overweight, and severe obesity in adults and children; emerging treatment opportunities; the development of a workforce for obesity treatments; payment and policy considerations; and promising paths to move forward. This publication summarizes the presentations and discussions from the workshop.

Obesity and non-communicable diseases (NCDs), significant threats to population health, are widely understood to be embedded in complex systems of interdependent causal factors. As such, researchers, policymakers and practitioners have become increasingly interested in systems-wide approaches that have the potential to reduce the burden of these diseases. Outcomes of this trend include the development and application of new systems science methods, and a turn towards multi-sectoral collaborative engagement as a key directive for influencing systems. This dissertation explores these aspects of the whole systems approach to obesity and NCDs through three original research papers. In the first, a novel systems science framework is employed to analyze recommendations drawn from 12 documents aimed at influencing obesity planning. Results show that many of the documents focus on changing the determinants of energy imbalance and lack planning at higher levels of system function, such as interconnections between system elements and goal setting. This paper demonstrates the utility of systems science frameworks for introducing systems thinking into policy-level planning in a manner accessible to public health stakeholders. The second and third papers turn to the subject of multi-sectoral partnerships. The first of these represents a review of the role of public health partnerships with the private sector in addressing obesity and NCDs. Contemporary challenges around working with the food and beverage sector are considered through a systems-informed lens that pushes traditional thinking about conflict of interest and the role of monitoring and evaluation activities related to partnership engagement. The following chapter presents an exploratory qualitative study of federal governmental public health staff's experiences working to develop co-funded multi-sectoral partnerships. Findings highlight the opportunities and challenges that emerge from government efforts to shift relations with traditional and novel partners in an effort to leverage partnerships for system change. Suggestions for how program implementers can take dynamic system attributes such as capacity, trust, and power relations into account when implementing multi-sectoral partnership programs are also offered. Finally, this dissertation concludes with a critical reflection on the research findings in light of the whole systems imperative and its implications for the public health response to obesity and NCDs.
