Excerpt from Medical Diseases of the War I have had constant opportunities of studying every phase of the various diseases occurring on active service, first as physician and neurologist to a number of military hospitals in London and to the New Zealand Hospital at walton-ou-thames, then as a member of the Medical Advisory Committee for the Prevention of Epidemic Disease in the Mediterranean Expeditionary Force in Lemnos, and subsequently in Salonica, where I also acted as Consulting Physician to the British Forces. This book is a record of my own observations, amplified by a study of the literature so far as time and opportunity have allowed. I have read most of the papers bearing on the subjects dealt with, which have been published in the English medical journals, together with a number of the more important French contributions and a small number of those which have appeared in the German language. The principal references are given at the end of each chapter. I should like to add, for the benefit of those who are more familiar with my former name of Hertz than with my new name of Hurst, that I have made the change because under present conditions it is natural for one of English birth and English descent for several generations to be unwilling to retain a German name. Excerpt from Medical Services, Vol. 1: Diseases of the War The chapters of the volumes on the Diseases of the War have been prepared by officers who held regular, territorial force or temporary commissions in the Royal Army Medical Corps, and who had special knowledge and personal experience of the diseases about which they write. The material contained in official documents, supplemented by the numerous references appended to each chapter, has been at their disposal. In the present volume a considerable amount of repetition will be found, notably in the chapters on in uenza and purulent bronchitis, consequent upon these diseases having been considered from two separate standpoints, namely, the experience of the epidemic amongst the troops in France and the experience of the epidemic in the United Kingdom. The second volume will contain chapters on nervous disorders, venereal and skin diseases, in addition to chapters on the medical aspects of aviation, gas warfare, and mine gas poisoning. Although these latter subjects have a wider significance than that of actual disease and might of themselves have formed a separate volume, it has been found convenient to introduce them into the volumes on the Diseases of the War. The measures for preventing disease, and the methods and results of laboratory research are fully detailed. In the volumes which will be published separately on Hygiene and Pathology during the War, and reference must be made to them for fuller information on these subjects. But it has been considered advisable to introduce a certain amount of detail with regard to preventive treatment and pathology into the present volumes. Excerpt from Memoranda on Some Medical Diseases in the Mediterranean War Area, Vol. 1: With Some Sanitary Notes This disease has occurred at Salonica within recent years. In hot, dry and sandy countries it is apt to appear in the season when dust-storms are prevalent. Etiology. The causal micro-organism is the Gram-negative Diplococcus intracellularis of Weichselbaum found in the cerebro-spinal fluid and meninges of persons suffering from the disease. It has been isolated from the blood and from the nasal and lachrymal secretions and is frequently demonstrable in the throat secretions of contact cases. It would seem that the healthy "carrier" is the chief source of infection and, harbouring the meningococcus in his throat, he distributes it when he coughs, spits or sneezes. Infected fomites, such as handkerchiefs, etc., may play a part and it is possible that food may become contaminated and infection take place through the gastro-intestinal tract. The general view is that it enters the brain either via the lymphatics through the cribriform plate of the ethmoid or the sphenoidal sinuses, or reaches brain or spinal cord by the blood through the respiratory passages. Observations on a regimental epidemic showed that: - 1. There are many more contacts than actual cases. 2. The cocci usually disappear quickly from the naso-pharyngeal mucus but may persist for 4 weeks. 3. They are most numerous and virulent during their first week of residence in the throat. 4. They are little resistant to the sun's rays. The prevalence of naso-pharyngeal catarrh favours infection and so do over-fatigue, over-crowding and bad ventilation. It is conceivable that under war conditions flies might spread infection. The whole of the "carrier" question is still subjudice. Symptoms. Incubation period uncertain but probably less than 10 days. The onset is very sudden, so much so that the patient may rapidly lose consciousness. If he can walk his peculiar gait may betray him for he looks as if he were going to throw himself backwards. He complains of headache, stiffness in the neck and chilly sensations. There may be a rigor and vomiting and the temperature is raised. The face is flushed, the mind confused, the patient possibly delirious. Extraction of the head is marked. It may be difficult to bend it forward. Herpes on the lips or elsewhere is common. The tongue is dry and furred. Strabismus may be noted and photophobia is frequent. The presence of Kernig's sign is a great help in diagnosis for it is rarely absent. Excerpt from War With Disease The first three editions of this book were sold almost exclusively to the army at home and abroad, while the fourth was bought largely by the public. Recently, a demand coming from officers and men of the Territorial Army enables me to bring out a fifth edition of War with Disease. The book has now been sold in every country where the British flag flies, the frequent orders from India being specially gratifying. I have to thank the many commanding officers who have paid me the compliment of having parts of the lectures read out regularly to their men. Since I first delivered the lectures contained in this little book, I have had the honour of repeating them to the officers of the Staff College in Camberley. For this audience I went a little more deeply into the scientific reasons for the suggestions I made. Were I to redeliver the lectures, I could in many cases change recommendations into reports of the actual results of the effects produced by carrying out the plan of campaign recommended in War with Disease. These results are ia-all respects most gratifying. Indeed, in more than one case commanding officers have been able to inform me that the result of acting upon the principles laid down has been a clean bill of health for the regiment, and statistics prove how greatly the health of the Army, as a whole, has improved within the past two years. I do not attempt to take any credit for this, but as one who has taken some part in directing the fight against disease, the fact that the fight is a winning one gives me very great pleasure.
